I know all too well the desire for that “magic” solution where all the challenges of obesity disappear overnight and you can achieve your goals effortlessly. Unfortunately, that’s not the reality for most people.

In this blog, I’ll dive into why some people experience slower results than others and clarify why terms like "non-responders" or "hypo-responders" might not be helpful if you haven't first addressed key lifestyle factors like hydration, protein intake, and sleep. What to Expect When Starting GLP-1 Medications

If you’ve recently started a GLP-1 medication like Wegovy, Ozempic, or the newer tirzepatide medications such as Zepbound or Mounjaro, you might be hoping for immediate, dramatic results. Many people anticipate that the medication will work like magic, eliminating their struggles with obesity overnight. However, the reality is often different.

For many, the initial results might be less dramatic, but you might notice some benefits like reduced hunger and fewer cravings. From a weight loss perspective, a reasonable goal is to aim for at least a 5% reduction in total body weight within the first 12 weeks. This is a benchmark for progress and helps determine if the medication is working effectively for you.

In practice, achieving this 5% weight loss within 12 weeks can be crucial not just for your health but also for continued insurance coverage. If you're not seeing at least a 5% reduction, it may signal a need for adjustments in your treatment plan. Insurance companies often require this level of progress to continue covering the medication, so it’s important to stay on track.

To calculate your weight loss percentage, divide your current weight by your starting weight. For example, if you started at 200 pounds and are now 180 pounds, you’ve lost 10% of your body weight. Monitoring your progress month-to-month can also be helpful. Ideally, you should aim to lose no more than 1% of your body weight per week, or around 4% per month.

While it’s common to hope for quick fixes, managing expectations and understanding the gradual nature of weight loss with GLP-1 medications can help you stay on track and make necessary adjustments along the way.

What If You're Seeing Less Than 5% Weight Loss?

If after the first three months on your GLP-1 medication you find that your weight loss is less than 5% of your body weight, it can be disheartening. Research suggests that a small percentage of people might be classified as “non-responders,” but in my clinical experience, this group is much smaller.

It’s important to remember that initial setbacks don’t mean your medication isn’t working. Often, the issue might be related to factors beyond the medication itself. To get the most out of your treatment, ensure you're following key lifestyle guidelines. For example:

  • Are you getting your hydration in? 
  • Are you hitting your fiber goals?
  • Are you prioritizing protein? 
  • Are you maintaining regular physical activity? 
  • What does your sleep look like and how are you managing stress?

I see patients frequently during the early stages of medication use to help them establish these habits. Waiting until the 12-week mark without addressing these factors can mean missing crucial opportunities to adjust and improve your results.

Before labeling yourself as a non-responder, consider whether you’re fully committed to these lifestyle changes. Yes, it can seem like a lot of work initially, but it’s essential for achieving the results you want.
